- 51,021 onward referrals for financial, housing, and mental health support This report is both timely and inspiring — a powerful example of how bridging clinical care with community support can transform outcomes for cardiac patients and their families. By tailoring support to individual needs, from financial and housing assistance to mental health services, the program goes beyond traditional clinical care. The measurable improvements demonstrate how social prescribing and precision health come together to deliver personalized, outcomes-focused care that addresses both medical and social determinants of health.
Next month, Gareth Presch, CEO, Global Social Prescribing Alliance will be sharing global best practices in social prescribing at the Radical Health Festival in Helsinki, and later this year at our GSPA Summit. The learnings from this collaboration continue to shape how we support patients holistically, showing the incredible potential of social prescribing on a global scale.
