For decades, the dominant international healthcare apparatus has operated under a reactive “sick-care” model an unsustainable architecture that stands by until an individual falls ill before deploying costly clinical resources. Today, facing a projected international shortfall of 10 million frontline workers by 2030 alongside soaring multi-morbidities, that model has hit its structural limit.

Today, during the historic Global Social Prescribing Alliance Summit 2026, we are proud to formally publish the Global State of Social Prescribing Report (2025–2026 Edition). This document serves as the definitive, unalterable archive of how an independent health movement mobilized an international ecosystem to make “prescribing a life, not just medicine” a scalable reality.
The Journey So Far: From a 2021 Blueprint to a Global Reality
When the GSPA was launched in 2021 by the UK Secretary of State for Health and Social Care alongside the National Academy for Social Prescribing (NASP), the World Health Organization (WHO), and the UNGSII Foundation, we set out to build a standardized, open architecture for global health delivery.
We structured our work across eight core non-clinical domains, spanning green and blue social prescribing, arts, physical movement, and immediate practical welfare support. Simultaneously, we mobilized the future generation of clinicians through our International Student Champion Scheme, embedding preventative medicine directly within university curricula across the UK, Australia, Japan, Portugal, and the United States.
Our geographic footprint stands as a testament to this collaborative sweat equity. The GSPA framework is now actively engaging with, developing frameworks for, and scaling community health programs within over 40 nations across Europe, the Americas, South and East Asia, Oceania, Africa, and the Middle East.
Anchoring the Architecture on the World Stage
This paradigm shift did not happen in a vacuum. GSPA and WHIS Founder Gareth Presch has systematically carried this blueprint to the highest diplomatic and economic platforms on earth, cementing social prescribing as a non-negotiable pillar of future societal prosperity.
From the floor of the United Nations General Assembly (UNGA) in New York to the panel rooms of the World Economic Forum (WEF) in Davos, our co-authored frameworks have challenged traditional definitions of health metrics. The alliance’s architectural principles were carved into urban planning agendas at the 11th World Urban Forum (WUF) and structurally linked to climate-resilient wellness systems during global text negotiations at COP28. Furthermore, these models reflect strategic insights directly generated during leadership contributions to Pope Francis’s Vatican COVID-19 Commission.
The Finance Revolution: Shifting Capital Upstream
A movement cannot survive on goodwill alone; it requires a permanent, independent funding stream. Historically, preventative health health initiatives have been choked by short-term, siloed, and highly volatile programmatic grants.
To solve this, our framework has integrated the World Health Service (WHS)—a sophisticated financial and governance vehicle operating as a component fund of Legacy Global Programs (a qualified 501(c)(3) public charity). By utilizing an innovative blended capital allocation mechanism, WHS bridges the gap between field-level health networks and institutional scale capital.
Through flexible Fiscal Sponsorship (FSP) and Donor-Advised Fund (DAF) pathways, WHS allows institutional investors, corporate partners, and philanthropists to direct capital upstream. This creates place-based, audited social and economic healthcare returns that systematically reduce the long-term strain on public health systems.
A Direct Call to Action for Sovereign Governments
The empirical data contained within our newly released 2025–2026 report is undeniable: evaluations demonstrate a 12% increase in client mental health self-management, a 49% decrease in chronic isolation metrics, and a proven social return on investment (SROI) of up to £36 for every £1 invested in WHIS-supported tracks.
The infrastructure is built. The financial vehicles are active. The clinical standards are set.
